Russia's industrial safety watchdog, Rostekhbadzor, said Monday that last month's accident at a Siberian hydroelectric power station was caused by failures in safety and working procedures, Russian news agency RIA Novosti reports on its Web site.
Rostekhbadzor said the causes and those being held personally responsible for the accident would be officially named Tuesday.
